New Patients

care, have fewer emergency room visits and fewer absences from work and school.

diagnosed and treated. Whether you see a doctor frequently or rarely have medical
problems, it is important to have a health care home. This is important for preventing
chronic illness, keeping immunizations and screenings up-to-date and coordinating
care with specialty medical providers. Having a health care home can improve your
health outcomes and quality of life.
Family Care Health Centers wants to be your health care home. You and your family
will get a full range of primary health care services at our centers. Plus, these
services will be affordable and easy to access. We have two state-of-the-art facilities
in the St. Louis metropolitan area to serve you.
By the end of 2011, all Family Care providers will begin using an Electronic Medical
Record.
